What Is NAD+ and Why Does It Matter?

Nicotinamide adenine dinucleotide (NAD+) is a coenzyme found in every living cell. It plays a central role in energy metabolism, DNA repair, and cellular communication. NAD+ levels decline with aging across multiple experimental models, but the extent and consistency of age-related NAD+ decline in humans remain less clearly established and appear to vary by tissue, population, and measurement method. What Is Nicotinamide Riboside (NR)?

Tru Niagen® uses a patented form of Vitamin B3 called Nicotinamide Riboside (NR), which serves as a precursor to NAD+. NR has been evaluated in multiple published human clinical trials and has consistently demonstrated the ability to raise NAD+-related biomarkers in the blood. Whether this biochemical target engagement translates to clinical health benefits is an active and ongoing area of research.

What the Clinical Research Shows

Tru Niagen® Pro 500mg contains Niagen® (nicotinamide riboside chloride), an NR ingredient that has been used in published human clinical studies. Key findings from the NR human research literature include:

  • Increased NAD+ biomarkers: Multiple human studies have documented that oral NR raises NAD+-related metabolites in a dose-dependent manner. The degree of increase varies by study population, dose, and duration.
  • Safety and tolerability: A randomized double-blind placebo-controlled trial (Conze et al., 2019, PMID: 31278280) found that NR at doses of 100, 300, and 1,000 mg/day for 8 weeks produced dose-dependent increases in whole-blood NAD+ with adverse-event rates that did not significantly differ from placebo.   • Cardiovascular-related measures: A small randomized crossover trial (Martens et al., 2018, PMID: 29599478; n=24) found exploratory signals for possible reductions in systolic blood pressure and arterial stiffness with NR 500 mg twice daily for 6 weeks, but the trial was not designed to establish cardiovascular clinical benefit and results require confirmation in larger trials.
  • Mitochondrial function — null result: A randomized placebo-controlled trial (Dollerup et al., 2020, PMID: 31710095) in men with obesity and insulin resistance found that NR 2,000 mg/day for 12 weeks increased NAD+-related metabolites but did not improve skeletal-muscle mitochondrial respiration, content, or morphology compared with placebo. This null result is an important part of the NR evidence base.
  • Overall: A 2026 PRISMA-guided systematic review (Gallagher & Emmanuel, PMID: 41655607) concluded that NR and NMN reliably show biochemical target engagement but clinical outcomes remain heterogeneous, often null or endpoint-specific, and clinical effectiveness for anti-aging and wellness remains inconclusive.
