What Is Methylation?

Methylation is one of the most fundamental biochemical processes in the human body, occurring over one billion times per second across virtually every cell. It involves the transfer of a methyl group (one carbon atom bonded to three hydrogen atoms) to DNA, proteins, enzymes, and neurotransmitters — regulating everything from gene expression and detoxification to mood, cardiovascular health, and immune function.

When methylation is impaired, the downstream effects are wide-ranging: elevated homocysteine levels, impaired detoxification, mood dysregulation, cognitive decline, and increased cardiovascular risk. Supporting optimal methylation is one of the most impactful interventions in functional and preventive medicine.

The MTHFR Connection

A key enzyme in the methylation cycle is MTHFR (methylenetetrahydrofolate reductase), which converts folate into its active form, 5-methyltetrahydrofolate (5-MTHF). Genetic variants in the MTHFR gene — particularly C677T and A1298C — are remarkably common, affecting an estimated 40–60% of the population to varying degrees. Individuals with these variants have reduced ability to activate folate, making supplementation with pre-methylated B vitamins essential.

Standard folic acid supplements are ineffective for individuals with MTHFR variants because they cannot be properly converted. This is why the form of folate and B12 in a methylation supplement matters enormously.

Key Nutrients in Methylation Support

Metagenics® MethylCare™ is formulated with the active, bioavailable forms of the key methylation cofactors:

  • 5-MTHF (Methylfolate): The biologically active form of folate that bypasses the MTHFR enzyme entirely — effective even in individuals with MTHFR variants.
  • Methylcobalamin (Methyl-B12): The neurologically active form of Vitamin B12, superior to cyanocobalamin for methylation support and neurological health.
  • Pyridoxal-5-Phosphate (P5P): The active form of Vitamin B6, a critical cofactor in homocysteine metabolism.
  • Riboflavin (B2): Essential for MTHFR enzyme activity and the broader methylation cycle.
  • Betaine (TMG): Provides an alternative methylation pathway via the BHMT enzyme, supporting homocysteine remethylation independent of folate.

Homocysteine: The Cardiovascular Risk Marker

Elevated homocysteine is a well-established independent risk factor for cardiovascular disease, stroke, cognitive decline, and all-cause mortality. The methylation cycle is the primary pathway for homocysteine remethylation back to methionine. Clinical studies consistently demonstrate that supplementation with methylated B vitamins — particularly 5-MTHF, methylcobalamin, and B6 — significantly reduces homocysteine levels:

  • A meta-analysis of 12 randomized controlled trials found B-vitamin supplementation reduced homocysteine by an average of 25%.
  • The VITATOPS trial demonstrated that B-vitamin supplementation reduced the risk of major vascular events in high-risk patients.
  • Research shows that for every 5 μmol/L reduction in homocysteine, cardiovascular risk decreases by approximately 18%.

Beyond Cardiovascular Health: The Broader Benefits of Methylation Support

  • Cognitive function: Methylation is required for the synthesis of neurotransmitters including serotonin, dopamine, and norepinephrine. Impaired methylation is associated with depression, anxiety, and cognitive decline.
  • Detoxification: Phase II liver detoxification relies heavily on methylation to neutralize toxins, heavy metals, and hormones.
  • DNA repair: Proper methylation patterns regulate gene expression and protect against epigenetic dysregulation.
  • Pregnancy support: Adequate methylfolate is critical for neural tube development and is particularly important for women with MTHFR variants.

Why Metagenics® MethylCare™?

Metagenics® has been a leader in practitioner-grade nutritional science for over 35 years. MethylCare™ uses exclusively active, methylated forms of all B vitamins — not the cheaper, inactive forms found in most commercial supplements. Each tablet is manufactured in NSF-certified facilities with rigorous identity, potency, and purity testing.

For patients with confirmed MTHFR variants, elevated homocysteine, or a history of cardiovascular disease, MethylCare™ represents one of the most targeted and evidence-based interventions available without a prescription.

Who Should Consider MethylCare™?

  • Individuals with known MTHFR gene variants (C677T or A1298C)
  • Adults with elevated homocysteine levels
  • Those with a personal or family history of cardiovascular disease or stroke
  • Individuals experiencing mood disorders, brain fog, or cognitive decline
  • Women planning pregnancy, particularly those with MTHFR variants
  • Anyone seeking comprehensive methylation and B-vitamin support

Pharmacist's Note

Methylation support is one of the most clinically impactful areas of nutritional medicine, yet it remains underappreciated in conventional care. At KHANG PHARMACY, we recommend Metagenics® MethylCare™ as our first-line methylation formula because it uses the right forms of every nutrient at clinically meaningful doses. If you've been told you have an MTHFR variant or have elevated homocysteine, this is the supplement we'd reach for first.

We strongly recommend testing homocysteine levels and MTHFR status before and during supplementation. Please consult your pharmacist or physician to interpret your results and personalize your protocol.
