The Clinical Question
Metagenics® UltraMeal® Cardio 360° is a practitioner-distributed medical food formula positioned by Metagenics for the nutritional management of dyslipidemia. Its distinguishing features include plant sterols as Reducol® (900 mg/serving; 1,800 mg/day at the labeled twice-daily regimen), alpha-linolenic acid from flaxseed, hops polyphenols standardized to xanthohumol, a complete plant-based protein blend, and a comprehensive cardiovascular-relevant micronutrient panel including active B vitamin forms.
This article reviews what published evidence supports for the key active components — plant sterols in particular — and clearly distinguishes between (1) ingredient-level meta-analysis evidence, (2) published RCT evidence using older UltraMeal-family formulations, and (3) the current Cardio 360° product itself, for which no independent published RCT of the current formulation has been identified.

What the Evidence Does — and Does Not — Establish
- Strongest evidence (ingredient level): Plant sterols have extensive meta-analysis support for LDL-C reduction at approximately 2 g/day, including additive effects when combined with statin therapy.
- Formulation-adjacent evidence: An older UltraMeal Plus 360° formulation was studied in a published RCT within a structured dietary program and showed improvements in lipid and metabolic markers beyond diet alone. This is not the current Cardio 360° product.
- Not established: That the current UltraMeal® Cardio 360° formulation itself produces LDL reduction or other cardiovascular endpoints in a published randomized controlled trial. Ingredient-level evidence does not substitute for a finished-product trial.
Cardiovascular Risk and the Nutritional Dimension
Elevated LDL cholesterol, low HDL, high triglycerides, insulin resistance, hypertension, and systemic inflammation are modifiable cardiovascular risk factors with established dietary components. Plant sterols, low-glycemic-load dietary patterns, and adequate protein intake each have a published evidence base for one or more of these risk factors. The strength and specificity of that evidence varies by factor, and dietary interventions do not replace pharmacological therapy where clinically indicated.
Ingredient-by-Ingredient Evidence Review
1. Plant Sterols (as Reducol®) — 900 mg/serving | 1,800 mg/day at labeled twice-daily regimen [Strongest evidence in this formula]
Plant sterols are structurally similar to cholesterol and competitively inhibit cholesterol absorption in the small intestine, reducing circulating LDL-C. This mechanism is well established and supported by extensive clinical trial data. Reducol® is a registered trademark of Pharmachem Laboratories; the 900 mg per serving includes 700 mg beta-sitosterol.
- FDA regulations permit certain health claims relating plant sterol/stanol ester consumption, when consumed as part of a diet low in saturated fat and cholesterol, to reduced risk of coronary heart disease. This regulatory framework should be distinguished from clinical evidence demonstrating LDL-C lowering.
- SYSTEMATIC REVIEW / META-ANALYSIS: A meta-analysis of 84 RCTs comprising 141 treatment arms (Demonty et al., PMID: 19091798) found that at a mean phytosterol intake of approximately 2.15 g/day, pooled LDL-C reduction was approximately 8.8%. This evidence supports approximately 8–10% LDL reduction at around 2 g/day — not the broader “8–15%” range sometimes cited.
- SYSTEMATIC REVIEW / META-ANALYSIS: Musa-Veloso et al. (PMID: 21345662; 113 publications, 182 strata) modeled maximal LDL reduction at approximately 8.3–8.4% for plant sterols/sterol esters, with plant stanols producing somewhat larger modeled maximal reductions at higher doses.
- SYSTEMATIC REVIEW / META-ANALYSIS — statin addition: Scholle et al. (PMID: 20439548; 8 randomized trials, n=306 statin-treated patients) found that adding plant sterols/stanols to statin therapy reduced LDL-C by an additional 13.26 mg/dL on average versus control. Han et al. (PMID: 27539156) independently supports additional lipid lowering in statin-treated patients. Important caveat: these studies evaluated lipid endpoints; they do not establish additional cardiovascular-event reduction beyond statin therapy alone.
- Dose note: At the labeled twice-daily regimen, Cardio 360° provides 1.8 g/day of plant sterols — close to, but not identical to, the approximately 2 g/day mean intake in the major meta-analyses. The evidence base should not be treated as directly establishing equivalent efficacy at 1.8 g/day. Discuss appropriate dosing and regimen adherence with your healthcare practitioner.
2. Alpha-Linolenic Acid (ALA) from Flaxseed — 400 mg/serving
ALA is a plant-derived essential omega-3 fatty acid. It undergoes limited conversion to EPA and DHA in humans. Evidence for ALA specifically in cardiovascular outcomes is weaker than for long-chain marine omega-3s (EPA/DHA). The 400 mg dose per serving is modest; evidence for cardiovascular benefit from ALA at this dose is limited.
3. Rice Protein Matrix & Hops Polyphenols (Xanthohumol 2.5%) — 250 mg/serving
Xanthohumol is a prenylated chalcone polyphenol from hops with preclinical evidence for antioxidant, anti-inflammatory, and metabolic effects. Human clinical evidence for xanthohumol at this dose for cardiovascular or lipid outcomes is limited; most supportive evidence is preclinical or mechanistic.
4. Low Glycemic Nutritional Profile
Cardio 360° is formulated with a low-glycemic nutritional profile (10g total carbohydrate, 6g fiber, 1g sugar, monk fruit sweetener, 0g added sugars per serving). Low-glycemic-load dietary patterns have been studied for effects on glucose, insulin, and cardiometabolic outcomes; however, the glycemic response to the finished Cardio 360° formulation has not been independently established in a published clinical study.
5. Protein Blend — 13 g/serving
Pea protein isolate and rice protein concentrate provide a plant-based, dairy-free, soy-free protein profile. Meta-analyses suggest that higher-protein diets may produce modest reductions in blood pressure and some cardiometabolic markers (Rebholz et al., PMID: 23035142; Vogtschmidt et al., PMID: 34120735). These findings represent general dietary-protein evidence and should not be interpreted as evidence that Cardio 360° itself lowers blood pressure.
6. Cardiovascular-Relevant Micronutrients
Cardio 360° provides calcium L-5-methyltetrahydrofolate and methylcobalamin, which participate in one-carbon metabolism and homocysteine metabolism. Their presence in this formula does not establish that Cardio 360° improves cardiovascular outcomes. The formula also provides magnesium (80 mg), chromium (120 mcg), and a comprehensive vitamin and mineral panel. The presence of individual micronutrients in the formula does not establish that Cardio 360° produces measurable cardiovascular outcomes.
7. UltraMeal-Family Formulation RCT (Formulation-Adjacent Evidence)
- HUMAN RCT: Jones JL et al. (PMID: 21600524) — 12-week randomized trial in women with metabolic syndrome; Mediterranean-style low-glycemic-load diet alone versus same diet plus UltraMeal Plus 360°. The medical-food group showed greater improvements in LDL-C, non-HDL-C, apoB, apoB/apoA1 ratio, and homocysteine versus diet alone. This studied UltraMeal Plus 360°, not the current Cardio 360° formulation, within a structured dietary program.
- HUMAN RCT: Lerman RH et al. (PMID: 18983673) — modified Mediterranean-style low-glycemic-load diet plus phytochemical medical-food intervention in metabolic syndrome/hypercholesterolemia subjects. Supported lipid and metabolic improvements within the structured program. Same formulation-transfer caveat applies.
Who May Consider UltraMeal® Cardio 360°?
- Adults with elevated LDL cholesterol or dyslipidemia whose healthcare provider recommends dietary intervention incorporating plant sterols
- Patients with metabolic syndrome, insulin resistance, or pre-diabetes seeking a structured low-glycemic medical nutrition option
- Those with elevated cardiovascular risk working with a healthcare provider on a comprehensive nutritional program
- Patients on statin therapy interested in additive LDL reduction through plant sterols — discuss with your pharmacist regarding sterol dose and any drug interactions
- Not a replacement for pharmacological therapy where clinically indicated; must be used under healthcare provider supervision as a medical food
Pharmacist’s Perspective
As a PharmD, I consider the plant sterol component the primary evidence-based feature of Cardio 360°. The Demonty et al. meta-analysis, Musa-Veloso dose-response analysis, and Scholle statin-addition meta-analysis provide a substantial ingredient-level evidence base for the LDL-C-lowering effects of plant sterols at approximately 2 g/day. I present this to patients as a structured medical nutrition tool with a meaningful ingredient evidence base — not as a proven cardiovascular drug or a standalone replacement for lipid-lowering therapy.
The current Cardio 360° formulation has not been studied in a standalone published RCT; the older UltraMeal Plus 360° trial (Jones et al., PMID: 21600524) provides the closest formulation-adjacent evidence, studied within a structured dietary program. I discuss this distinction explicitly with patients before recommending the product. I also clarify that the labeled twice-daily regimen supplies 1.8 g/day of plant sterols — close to, but not identical to, the approximately 2 g/day mean exposure in the phytosterol meta-analyses — and that adherence to the twice-daily regimen is important for approaching the studied dose range.
