What Is MegaFood® Turmeric Curcumin Extra Strength?
MegaFood® Turmeric Curcumin Extra Strength is a multi-ingredient liver and anti-inflammatory support formula. Each 2-tablet serving delivers a turmeric/curcuminoid blend alongside milk thistle (silymarin), schisandra berry, and BioPerine® black pepper extract. This article reviews the clinical evidence for the key active ingredients, clarifies what the research does and does not support, and explains how I discuss this formula with patients at Khang Pharmacy.

Formulation & Bioavailability Note
Curcumin has poor oral bioavailability in standard formulations and is rapidly metabolized. This product addresses that limitation with two complementary strategies: (1) BioPerine® (piperine), which has been shown to enhance curcumin absorption by inhibiting intestinal and hepatic metabolism; and (2) inclusion of both whole turmeric root and a concentrated 95% curcuminoid extract in the same serving.
Important dose context: Per the Supplement Facts panel, MegaFood® provides 300 mg of turmeric root extract standardized to 95% curcuminoids (~285 mg curcuminoids from the extract), plus 500 mg whole turmeric root, and 3 mg BioPerine® per serving. Many curcuminoid clinical trials use higher curcuminoid doses or different delivery systems. Studies using substantially higher doses or different formulations cannot be assumed to apply directly to this product. This does not mean the product is ineffective — it means direct dose extrapolation is not appropriate.
Multi-Ingredient Disclosure: Milk Thistle & Schisandra
This formula also provides meaningful amounts of two additional hepatoprotective botanicals:
- Milk thistle seed extract (225 mg, 80% silymarin): Silymarin has its own clinical evidence base for liver protection, including randomized trials in NAFLD, hepatitis, and drug-induced liver injury. It is also a known modulator of CYP3A4, UGT enzymes, and P-glycoprotein, which has drug-interaction relevance for patients on medications metabolized by these pathways.
- Schisandra berry extract (100 mg): Schisandra has hepatoprotective properties supported primarily by preclinical and limited human research. It has been identified as a CYP3A4 and P-gp modulator. Patients on medications with narrow therapeutic indices should be aware of this ingredient when reviewing supplement-drug interactions with their pharmacist.
The evidence for milk thistle and schisandra as individual ingredients is not reviewed in depth in this article. Their presence meaningfully expands the interaction and clinical profile of this formula beyond a standard curcumin supplement.
The Clinical Evidence for Curcumin/Turmeric
- Knee osteoarthritis: A 2021 systematic review and meta-analysis of 11 studies involving 1,258 participants (PMID: 34537344) found curcuminoid supplementation improved knee osteoarthritis pain outcomes compared to control. Notably, the analysis found no significant difference in pain relief between its defined high-dose (≥1,000 mg/day) and low-dose (<1,000 mg/day) groups. Formulations and doses varied substantially across included trials. These findings should not be interpreted as evidence that MegaFood® Turmeric Curcumin Extra Strength produces equivalent effects or is equivalent to established osteoarthritis therapy.
- NAFLD / liver health: A 2022 systematic review and meta-analysis of 16 randomized trials involving 1,028 participants (PMID: 35661765) found curcumin supplementation improved ultrasonographic measures of NAFLD severity and steatosis, and produced modest reductions in ALT and AST, compared to placebo. Considerable variation in formulations, doses, populations, and study designs limits direct extrapolation to any specific curcumin product. A randomized controlled trial (n=55, PMID: 32147075) using 500 mg curcuminoids + 5 mg piperine daily for 8 weeks found improvement in ultrasound NAFLD severity and several inflammatory markers including TNF-α, MCP-1, and EGF vs. placebo. This trial is conceptually relevant to MegaFood® because it combined curcuminoids with piperine, the same approach used in this formula — though the trial used higher amounts (500 mg curcuminoids, 5 mg piperine) than MegaFood® provides (~285 mg curcuminoids from standardized extract, 3 mg piperine). Same concept does not imply same pharmacokinetics or clinical outcome. Curcumin should be considered adjunctive rather than established treatment for NAFLD.
- Metabolic health: Human trials have examined curcumin for effects on fasting glucose, insulin sensitivity, triglycerides, and inflammatory markers in metabolic syndrome populations. Results are directionally positive in some studies but variable by population, dose, formulation, and follow-up period. Evidence is insufficient to establish curcumin as standard metabolic therapy.
Who May Consider Discussing This Formula?
- Adults with osteoarthritis or chronic joint discomfort interested in discussing adjunctive nutritional approaches with their pharmacist or physician
- Individuals with elevated liver enzymes or NAFLD under physician management who are interested in adjunctive nutritional support
- Adults with metabolic syndrome or cardiovascular risk as an adjunct to established lifestyle and medical management
- Adults interested in a combined turmeric/liver-botanical formula rather than individual supplements
Safety & Medication Considerations
- Anticoagulants and antiplatelet therapy: Curcumin has demonstrated effects on platelet and coagulation-related pathways in experimental research, creating a theoretical potential for additive bleeding effects. Patients taking warfarin, DOACs, antiplatelet medications, or other agents that increase bleeding risk should consult their pharmacist or prescriber before use.
- Drug-metabolism interactions (CYP/P-gp): Curcumin, milk thistle (silymarin), and schisandra can all affect drug-metabolizing enzymes and transporters in experimental models. The clinical magnitude of many proposed interactions remains uncertain, but patients taking medications with narrow therapeutic indices or complex interaction profiles should review this multi-ingredient formula with a pharmacist before starting.
- Gallbladder disease: Curcumin stimulates bile production; patients with gallstones or bile duct obstruction should consult their physician before use.
- Pregnancy/nursing: Consult your healthcare provider before use. Insufficient safety data at supplemental doses for concentrated curcumin, silymarin, or schisandra during pregnancy.
- Allergy: Discontinue use and seek medical attention if you experience rash, hives, swelling, or difficulty breathing.

As a PharmD, I discuss MegaFood® Turmeric Curcumin Extra Strength with patients seeking a combined curcumin and liver-botanical formula, particularly those with osteoarthritis or NAFLD under physician management. The inclusion of BioPerine® is clinically relevant because piperine has been used alongside curcuminoids in human NAFLD research to enhance absorption; however, the studied formulation and doses should not be assumed to be equivalent to this finished product. The curcuminoid dose (~285 mg from the standardized extract, plus whole turmeric root) is lower than doses used in many positive trials, and I always disclose that gap. The presence of milk thistle and schisandra adds meaningful hepatoprotective support but also expands the drug-interaction profile, so I always review the patient’s full medication list — particularly for those on warfarin, DOACs, statins, or any narrow-therapeutic-index medication.
